Job Description

Summary of Position:

 Responsible for assisting the Director of Nursing and Administrator in the operations of the resident care department. Assists with training and orienting of new resident care staff. In times of need works on the floor as a Charge Nurse or Resident Care Aide. Assists and at times conducts regular on-going care staff meetings and in-services. As directed, assigns and delegates responsibilities to resident care staff for residents, based on the qualifications of the staff and needs of the residents. Ensures each resident receives services/care as per Resident Service Plan and in accordance with federal and state regulations and community policies and procedures. Helps to facilitate communication within the department as well as with other departments and Administration.   In the absences of the Director of Nursing, will assure prompt, appropriate action in cases of injury, illness, fire or other emergencies.   In the absence of the Director of Nursing performs assessment in coordination with Administrator and marketing staff members on each resident prior to admission to determine if prospective resident’s care needs can be appropriately met by the community. Communicates with Director of Nursing and Administrator about change is residents’ condition. Participates in resident service plan care conferences. Ultimately responsible for ensuring care department compliance with state and federal laws and community policies and procedures in the absences of the Director of Nursing.

Qualifications:

  •  Meets regulatory qualifications as specified by Hawaii Administrative Rules, Department of Health, Title 11, Chapter 90
  • Prefer current RN or LPN with license in good standing
  • At least one year of previous supervisory experience preferably in an assisted living setting
  • Maintains required continuing education hours relative to the position and maintenance of license
  • Must meet all employee requirements established in Employee Handbook and Operating Policies and Procedures Manual
  • Must meet all requirements established by regulation that pertain to employees who work in an Assisted Living setting
  • Professional in appearance and work habits
  • Possess effective leadership skills
  • Demonstrates integrity, imagination and initiative, showing appropriate manner and conduct
  • Ability to effectively direct, delegate and follow-up on programs and service/care of the residents
  • Excellent problem identification and resolution skills
  • Ability to effectively teach and/or make oral presentations to individuals and groups
  • Genuine interest in the needs of the residents served
  • Ability to read, write, speak and understand English
  • Effective writing and documentation skills
  • Self-motivated and well organized
  • Ability to relate positively, effectively and appropriately with residents, families, community members and all levels of personnel
  • Knowledgeable about federal and state rules regulations governing an assisted living community
  • Ability to relate well to persons of varying backgrounds, abilities and expectations
  • Ability to use and demonstrate use of all resident services equipment
  • Acceptable references
  • Physical requirements of position: continuous standing/walking/bending/stooping, frequent pushing/pulling/reaching above shoulders and lifting, occasional sitting. In addition, must be free of communicable disease and meet community policies and procedures regarding necessary physical health

Essential Functions:

      Supervisory

  • Identify situations that require counseling employees and communicating action need to the Director of Nursing or Administrator in a fair and consistent manner in accordance with policies and procedures.
  • Schedule resident care staff and assure correct number of staff is on duty.
  • Enforce the attendance policies and if needed, replace employees who call in sick or don’t show up for a shift.
  • Supervise care staff in the absence of the Director of Nursing and provide feedback towards the performance of staff on an ongoing basis to ensure residents receive excellent care/service.
  • Identify and communicate problems on care floor. Assist Director of Nursing and Administrator with a plan of action and follow through with specific problems on care floor.
  • Assist with assuring documentation on resident records by care staff is complete and accurate.
  • Communicate with care staff to provide information and address concerns.
  • Assist with on-going trainings/in-services with staff to teach appropriate charting and care methods.
  • Participate in meetings to keep staff in other departments informed of new developments in resident service/care, policies and procedures.
  • Instruct personnel on use of equipment as instructed.

Resident Care Oversight

  •  Provide assistance on functional assessments on all residents prior to admission and assess need for care services.
  • Engage resident and family participation in service/care planning by explaining approaches and involving them in teaching programs. Conduct (or ensure occurrence of) on-going care conferences/assessments as per policies and procedures with family members/resident representatives.
  • Make resident rounds frequently to assure that each resident is given timely and appropriate service.
  • Monitor residents’ physical and mental functioning, psychological well-being and nutritional status to ensure care needs are met in accordance with resident service plans, Department of Health requirements and community policies and procedures.
  • Notify Director of Nursing and Administrator, physician and family and take appropriate action as directed when significant changes in condition occur.
  • Ensure that Incident Reports are written properly in a timely manner and that appropriate follow-up investigation on incidents occurs and is documented on all shifts.
  • Ensure medications are ordered and received from pharmacy in a timely manner and handled in accordance with policy.
    • Ensure proper contact with physicians occurs regarding written and verbal orders (this includes properly transcribing physicians’ orders with accuracy and assuring follow-up as indicated) in accordance with regulations, standard nursing practice and established community policies and procedures.
  • Ensure each resident is treated with dignity and respect. Investigate complaints of residents and families and take appropriate action. Maintain confidentiality regarding communications about residents’ conditions and medical records.
  • Assist with making necessary and expedient arrangements for consultants, laboratory and x-ray as necessary.
  • Assist with development of and maintain quality assurance programs in coordination with Administrator, community staff and Management Company. Make recommendations regarding updating policies and procedures to keep care program in compliance with state and federal laws while offering the best possible care to residents.
  • Perform direct resident care as necessary and appropriate.

Other

  •  Ensure adequate supplies are maintained at resident care office. Submit requests for supplies per community system and within care department budget.
  • Assure resident service department supplies and equipment and work areas are clean, neat and orderly.
  • Complete forms and reports as requested by Director of Nursing and Administrator.
  • Participate constructively in meetings and committees as requested by Director of Nursing and Administrator.
  • Conduct and attend trainings/in-service meetings.
  • Maintain good public relations with families, visitors, community members, residents and other community personnel.
  • Respond quickly to all emergency situations. Assist in fire drills and control and evacuation during an actual fire and disaster. Assure care staff members are knowledgeable regarding roles in emergencies. Perform CPR and First Aid as necessary.
  • Work effectively and cooperatively with state inspectors that come into building regarding surveys and care complaints.
  • Perform other duties as directed by Administrator.
