Job Description

The Hawaii Pathways Project (HPP) Intensive Case Manager – Veterans (ICM) is a member of a multi-disciplinary team responsible for ensuring HPP consumers (identified as veterans who are chronically homeless and struggling with substance use disorders and/or serious mental illness) are provided with 24/7 intensive case management services that allow them to locate, acquire, and maintain permanent housing. These services include basic eligibility assessments, psycho-education (group and/or individual), coordination of medical needs, and coordination of appointments/referrals with community providers, housing placement assistance, home visits, conducting psychiatric and medical assessments, and managing and monitoring psychotropic medication administration as needed. The HPP is a voluntary program that focuses on the Pathways Housing First model, which incorporates housing with enhancement of the consumer’s strengths rather than their substance abuse/mental illness, symptoms, and psychiatric history.  The program will rely on the talents, skills, and abilities of all its team members in order to function effectively, and achieve the program goals.  The program is under the direct supervision of the Helping Hands Hawaii (HHH) Medical Director, HPP Team Leader (TL), and HHH Director of Behavioral Health, to ensure adequate staff to support a safe, structured, and therapeutic environment in which members and staff work. The ICM is to comply with the company code of ethics, confidentiality practices, HIPAA, safety and all other applicable policies and procedures.

 

Qualifications:

  • Bachelor’s degree in a mental-health related field including, but not limited to, counseling, psychology, social work, social welfare, or other behavioral science.
  • An exception may be made for the above-mentioned educational requirements, on an individual case-by-case basis, if a candidate meets the following combination of required minimum education & credentials:
    • RN or LPN license. May have a Bachelors or Associates degree in any field.
  • 1-1/2 years of specialized experience working with veterans.
  • Must have strong commitment to the right and ability of each person with severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services
  • Skills and competence to establish supportive trusting relationships with persons struggling with substance abuse and severe and persistent mental illnesses and respect for client rights and personal preferences in treatment are essential.
  • Valid Hawaii Driver’s license, clear traffic abstract and willingness to use properly insured vehicle.
  • Clear pre-employment reference check, annual TB Clearance, CPR, First Aid and CPI certification.

 

Preferred Qualifications:

  1. 1-1/2 years of specialized experience working within the homeless services sector and/or working with

adults with substance or co-occurring substance abuse/mental illness.
