Job Description

The Hawaii Pathways Project (HPP) Intensive Case Manager – Veterans (ICM) is a member of a multi-disciplinary team responsible for ensuring HPP consumers (identified as veterans who are chronically homeless and struggling with substance use disorders and/or serious mental illness) are provided with 24/7 intensive case management services that allow them to locate, acquire, and maintain permanent housing.
