Home & Community Based Services
Location: Honolulu, Hawaii

We are seeking an experienced Licensed Social Worker (LSW) to join our Home & Community Based Services team. The position is Full Time (40 hrs. per week) working 9am to 5pm Monday through Friday. The selected candidate will work in various locations including our adult day cares and in clients’ homes.

Key Responsibilities include but are not limited to:

  • Provides medically related social work services to help clients attain or maintain their highest practicable physical, mental, and psychosocial wellbeing
  • Ensures that all client rights and responsibilities are protected and enforced, and establishes and maintains a close liaison with clients and families
  • Provides initial and continuing assessment of the client in developing an individual plan of care in collaboration with the interdisciplinary team
  • Identifies and assists in appropriate discharge planning for clients
  • Provides clients and families with referrals to outside resources
  • Provides referral and follow-up when client is not appropriate for adult day health program
  • Keeps informed of prevailing Federal and State regulations relating to long term care and best practices in medical social services

The Successful Applicant must have:

  • Master’s degree (M.S.W.) in Social Work; and one to two years related experience and training; or equivalent combination of education and experience.
  • Current State of Hawaii licensure to practice social work as required by the Hawaii Revised Statues (HRS) 467E
  • Current, valid State of Hawaii driver’s license and clear driver’s abstract and access to a motor vehicle to provide service in various locations
  • Experience working with the elderly in a Long Term Care setting is preferred
  • Proficiency in using a computer including the Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and other applicable software applications
  • Flexibility to work other days/times as scheduled to attend mandatory training and at management’s discretion
